Fill out all the information and answer all of the questions, only one essay question!
ESSAYS
(Coherence of ideas and grammar are important!)
You must complete one of the three essay questions.  The first three questions below are
the essay questions.  The essay should be at least 400 words.  Then answer questions 4
through 8 don't forget to answer all of the questions.
1)  Describe a significant event that cause you to become committed to living for Jesus Christ!
2)   Zeta Phi Zeta, chapter's mission requires that you demonstrate a willingness to minister the Word of God to
all.  In what ways has your school, church, or community benefited from your spiritual growth?

3)  Please choose one: Identify how you would minister to someone on campus who might be experiencing one
of the following; Lack of family support; lack of financial aid; lack of academic preparation; adaptation to
academic and social environment; lack of self esteem; pregnancy; and inappropriate behavior (i.e. drug use
and/or selling of drugs give scripture references to support your answer

4)  What is your favorite scripture and why?

5)  What is the one thing that you personally struggle with?

6)  As a Christian, what are your views on secular music?

7)  Do you attend secular, non-school related parties?  Yes or no and why?

8)  What is your Church and Pastor's name?

Thank you for your interest in Undergraduate Membership into Zeta Phi Zeta, Inc.  The documentation listed is required by the National Board for membership
consideration and review.

Total Undergraduate Membership Fee $350.00.

Return all documents and a $25.00 Money Order to:
Zeta Phi Zeta, Inc., P.O. Box 195, Rowlett, TX  75030-0195


*Original Application with signed and dated New Membership Agreement.

*Original Anti-Hazing Statement with initials.

*The Undergraduate Questionnaire with your answer sheet attached.

*Original Letter of Recommendation from your Pastor.

*Complete all documents thoroughly and print legibly.

*A Money Order for $25.00. This is your application fee and is non-refundable.  Make it payable to Zeta Phi Zeta, Inc.  If accepted onto a grafting line, this fee will
count towards your total membership fee.

*Be sure to include the chapter name you are applying through.

*If you are at a school where there is not a Zeta Phi Zeta chapter, fill in   your school name where the chapter name would be.


After receipt of your application package:


*A chapter officer will contact you for an interview.  If accepted, then,

*You receive a Welcome Letter informing you of your acceptance onto the grafting line of the chapter.

*A chapter officer will contact you with the grafting line start date.

*The first $100.00 payment is due prior to the start date of the grafting line.  This payment can be made by Money Order or through Paypal on our website.
